Abirateron is een anti-androgeen dat sinds 2012 in Nederland wordt gebruikt bij castratieresistent gemetastaseerd prostaatcarcinoom en sinds 2018 ook bij hormoongevoelig prostaatcarcinoom. We presenteren een casus van een patiënt die recentelijk was gestart met abirateron en prednison vanwege een castratieresistent prostaatcarcinoom. Hij presenteerde zich kort na starten van de behandeling op de Spoedeisende Hulp met klachten van zwakte in de benen en malaise. Er bleek sprake van een ernstige acute nierinsufficiëntie als gevolg van rhabdomyolyse. Het klinisch beeld werd geduid als abirateron-geïnduceerde toxiciteit van rosuvastatine die patiënt reeds langere tijd gebruikte. Na initieel conservatieve behandeling werd dialyse opgestart en enkele weken gecontinueerd, tot uiteindelijk gedeeltelijk herstel van de nierfunctie optrad. We veronderstellen een medicatie-interactie via het transporteiwit OATP1B1 als oorzaak van de rhabdomyolyse.
